In the Optum360 Coding Corner, we highlight areas in which medical coding professionals are struggling for uniformity and correct coding amongst their peers. In this article, we discuss coding variances for ICD-10-CM, ICD-10-PCS and/or CPT® found on a monthly basis and explore the codes, coding concepts and conventions for those variances. We also highlight new coding information when applicable, to keep the coding community informed.

2018 ICD-10 and PCS Update

The Centers for Medicare & Medicaid Services has posted on their website the new code update for 2018. The codes are effective beginning October 1, 2017, through September 30, 2018. New guidelines have not been published at this time. The link below will take you to the CMS website.

Other Encephalopathy

When physician documentation states encephalopathy due to sepsis, it is important that coders assign the correct diagnosis. According to the Index, encephalopathy due to sepsis is assigned to G93.41 (Metabolic encephalopathy). When encephalopathy occur secondary to a lacunar infarction, the encephalopathy should be coded separately. According to Coding Clinic, 2nd quarter, 2017, the encephalopathy is not inherent to the infarction. Diagnosis code G93.49 (Other encephalopathy) is coded when it occurs at the time of a lacunar infarction.
