NUCC Seeks Approval for CMS-1500 Revisions

The National Uniform Claim Committee has submitted to the Centers of Medicare and Medicaid Services for approval revisions to the CMS-1500 claim form. The revisions would accommodate the electronic version 5010 837P format as well as ICD-10-CM implementation. In addition to allowing the longer ICD-10-CM code format, the revised, or February 2012 (02/12), version includes changes to sections related to patient and insured information, as well as provider of service or supplier information.

The NUCC has recommended that health plans, clearinghouses, and other support vendors are ready to accept the 02/12 version of the form by June 1, 2013. It is recommended that physicians and other providers use either the August 2005 (08/05) or February 2012 version of the form between June 1 and October 1, 2013, and that as of October 1, 2013, the February 2012 version be mandated for use.

The NUCC has posted the draft of the form. Since both the Office of Management and Budget and CMS must approve the form, the posted 02/12 version of the claim form does not contain OMB approval numbers. The unofficial version of the revised form is being released only for testing and transition preparation purposes and should not be used for the official purpose of claims submission at this time.
