2013 Changes to Psychiatric Services Codes

For 2013, all CPT psychotherapy codes in the medicine section will be replaced with codes that are “time-based.”

Currently, the psychotherapy codes are classified according to whether the services are rendered in the office or other outpatient facility or in the inpatient hospital, partial hospital, or residential care facility. The codes are then further categorized by the type of psychotherapy—insight oriented, behavior modifying, and/or supportive or interactive. New code descriptions will include an approximate amount of time the visit is expected to last.

The 2013 codes simply state the type of service as psychotherapy, with the patient and/or family member present and the session time: 30-, 45-, or 60-minute increments. Further, psychotherapy add-on codes have been created according to the same time increments for use when the physician or other qualified health care professional provides psychotherapy at the same session as an evaluation and management (E/M) service; this allows the provider to report the psychotherapy with the appropriate E/M code. Interactive psychotherapy is now a new add-on code (90785) that may be appended to the psychotherapy codes when the type of therapy is described as “interactive.” Two additional codes, one for the first 60 minutes and another for each additional 30 minutes, have been created to describe crisis psychotherapy.

Codes 90875 and 90876 (Individual psychophysiological therapy incorporating biofeedback training by any modality [face to face with the patient], with psychotherapy [e.g., insight oriented, behavior modifying or supportive psychotherapy]) have been revised to clarify the time period associated with the codes as being 30 and 45 minutes, respectively.
